Thank you for choosing Lillington Veterinary Hospital for your pet’s boarding needs! Before you drop off your pet, please complete our Boarding Intake Form to help us ensure your dog or cat enjoys a safe, comfortable, and stress-free stay.

How to Complete the Form

This form is quick and easy to fill out on any device. You’ll be asked for:

  • Your pet’s feeding schedule and dietary needs
  • Any medications, dosages, and administration instructions
  • Behavioral notes or comfort preferences
  • Emergency contact information
  • Special needs or health concerns

Please submit the form before dropping off your pet so we can prepare their accommodations and ensure a smooth check-in.

WELCOME! The staff at Lillington Veterinary Hospital welcomes your pet to their home away from home. We will care for your pet while you are away. Our normal accommodations include 3 walks a day in a fenced yard, fresh water and bedding each day. To do our best we need an understanding of their routine. PLEASE PREPACKAGE EACH MEAL IN INDIVIDUAL BAGS AND PROVIDE MEDICATION IN THE ORIGINAL PRESCRIBED MEDICATION BOTTLE.

BOARDING CHECK OUT IS BY 12PM. ADDITIONAL CHARGES WILL ACCRUE AFTER 12PM (including Sunday) PREPAYMENT IS REQUIRED FOR SUNDAY PICK UP.

Every hospitalized patient should be free of external parasites. If your pet is found to have fleas, a Capstar ($10.00) tablet will be given to eliminate them immediately.
Are you picking up in the morning before 12:00pm or in the afternoon after 2:00pm? (We are closed between 12:00pm-2:00pm during weekdays.)
On Saturdays our pick up time is morning only between 8:30am-11:45am and our only pick up time on Sunday is in the evening between 4:30pm-5:00pm.
INFORMED CONSENT FOR BOARDING
I understand that 24 hour supervision of animals is not ordinarily provided at this hospital. The staff at Lillington Veterinary Hospital is to use reasonable caution against injury and escape of my pet. Lillington Veterinary Hospital will not be held liable for problems that develop, provided reasonable care and precautions are followed. I understand any medical problems my pet develops while here will be treated as deemed best by the veterinarians after consultation with me (unless I am unavailable). I assume responsibility for any expenses incurred for treatment while my pet is boarding.

If I do not pick up my pet within 48 hours of the date listed without notice, you may assume my pet is abandoned and you are authorized to rehome them as deemed fit. I understand that payment will be due, in full, upon the discharge of my pet.
